Did that actually send something?

Yes. One message was written to the address you pasted, from Pager's postbox wallet. One per wallet, ever, so pressing again changes nothing.

Does sending create a transaction?

No. Messages stay offchain. Connecting takes one signature, and sending spends no SOL.

Can I page a wallet that has never joined?

Yes. That is what the composer above does. The message waits as a request until the owner connects that wallet.

What happens to the message if nobody ever connects?

It stays where it is. Messages are plaintext rows in Postgres, readable by that address's owner once they sign in.
